Job Description:

1.0 JOB DESCRIPTION

Position

Outreach Worker

2.0
POSITION OBJECTIVES

To implement GFATM R9 HIV AIDS project activities by establishing CHBC site working in collaboration with PRs, SRs and by using implementation and quality assurance plans and tools for the project.

3.0
JOB RESPONSIBILITIES, DUTIES AND TASKS

3.1 Core Responsibilities & Duties

1. Identification and Registration of PLHIVs

2. Provide care and support to PLHIV at the CHBC and in the Home.

3. Undertake field visits to verify the address of the person who has been enrolled on ART and send to the CHBC or counseling support

4. Undertake follow-up visits to ensure adherence to ART

5. Provide information to the community regarding the community care center and the services offered

6. Mobilize pregnant women for PPTCT services and liaison with the other health functionaries

7. Provide basic nursing care

8. Provide palliative care

9. Administering DOTS treatment for tuberculosis

10. Referring PLHIV & family

11. Educating PLHIV & family on basic nutrition, nursing care & treatment

12. Assist in arranging for PLHIV & family to receive welfare support such as food, blankets, clothing, transport & assistance with funerals

13. To assist in developing & implementing an annual, quarterly & monthly planning along with a budget according to strategic plan

14. To assist in developing an annual, quarterly & monthly review according to the plan

15. To submit the reporting format on a monthly

16. To file the important files (hard & soft copy) in a proper way for institutional memory.

17. To provide all types of reports, pictures IEC material to the resources center

18. To share information on a timely basis with the team

19. To participate in meetings & workshops & actively participate in discussion & decision making

20. To ensure effective interpersonal relations among the team & solve conflicts in time

21. To create a positive, friendly & gender sensitive environment within team

22. Establish rapport with the PLHIV community and NGOs working in the area

23. To assess the needs of community for linkages & to identify relevant Org for linkages

24. To facilitate the establishment of linkages of community with the identified Org

25. To develop partnerships with community Org, NGO�s & service providers

3.2 Occasional Significant Duties

  • Representation of program at different forums
  • Support in organizing project related events/meetings/workshops
  • Extensive travel in the project target areas for community and home based care to people living with HIV/AIDs
  • As assigned by Project Senior Officer

BA/BSc/ equivalent qualification

2-3 years experience in HIV AIDS

Skills
  • Community mobilization skills and data collection / data entry and documentation skills.
  • Presentation, communication skills
  • Basic Ms Office skills.
  • Fluency in Pashto, Urdu
