Job Overview:

To carry out services such as Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Annual Wellness Visits (AWV) on patients who are registered in the company’s system (clinics/facilities in the USA). Additionally, to coordinate with the clinic staff to ensure that patients’ concerns are communicated to the Primary Care Provider regarding matters such as symptoms, hospitalizations, appointments, and medication refills.

Responsibilities:

CCM: Enrolling patients in CCM by giving them an introduction to the program and taking consent.

  • Reading the notes written by the Clinical Manager for each patient assigned daily for monthly review, and carefully going over their charts including diagnosis, care gaps, and medications.
  • Making the Care Plan for each patient.
  • Calling the assigned patients and discussing their health concerns with them, including addressing the points emphasized by the Clinical Manager in the quick notes.
  • Asking assessment questions and assigning an intervention on every monthly review.
  • Ensuring that the encounter takes 20 minutes and documenting using the timer.
  • Write down the points discussed in the conversation under the encounter notes.
  • This includes symptoms, medication reconciliation, medication refill requests, appointment requests, counseling related to diet and exercise, and asking the patient about their care gaps such as colorectal screening, mammography, etc.
  • Checking the Task tab to carry out the tasks assigned by the Clinical Manager and coordinating with the clinic staff to ensure the tasks are carried out. RPM:
  • Contacting eligible patients into the RPM program by referring to the list that is made by the team and calling them or setting up an appointment at the clinic.
  • Enrolling patients by explaining the program to them and taking consent. Keeping an organized inventory of the RPM devices.
  • Setting up the RPM device for the patient in the clinic, or remotely.
  • Update the patient status in the list. Monitor readings sent by the patients every day.
  • Calling patients who fail to send readings and recording the encounter in the system.
  • Call patients who have abnormal readings and schedule them for an appointment if needed.
  • Helping the patient navigate the app and the device in case they have any difficulties.

AWV: Referring to the list of scheduled patients daily.

  • Carrying out the AWV on the patients who arrive for their appointment and documenting it in the system.
  • Ensuring that behavioral counseling for obesity, cardiovascular disease, alcohol, and tobacco use are carried out if applicable.
  • Performing End-of-Life Planning in patients who give consent.
  • Take vitals and BMI to record in the AWV form. Reminding the PCP to sign the completed AWVs.
  • Updating patient status in the list daily (Completed, pending, no show, etc.)

Requirements:

  • Must have excellent English skills (Verbal and Written)
  • Must have excellent knowledge of Medical Terminologies
  • Pharmacists and Dentists will be preferred on priority.

Job Timings: 6 PM to 2 AM
