Monthly follow up of diagnosed patients for treatment.
Mobilization and awareness sessions in the community
Weekly progress meetings with District Supervisor and District Coordinator
Regular reporting to District Supervisor in soft and hard form.
Community Health Officer
Major Tasks:
Daily visit onboard doctors and PPM (Public & Private Mix) Labs and Informal clinics.
Verification of registered patients and family screening at home and screening.
Monthly follow up of registered patients.
Data maintaining for (TB01, TB02, TB03, TB04, TB05, TB07, TB09, Stock Register etc.
Regular reporting to District Field Supervisor in soft and hard form.
Conducting community awareness session as per plan.
Conducting and facilitating chest camps as per approved plan.
Regular weekly and monthly meetings.
Health/Field Worker
Follow up and contact register of index patients.
Household visits for verbal screening and counselling of patients and caregivers.
Convincing and facilitation of target families for CXR at health facility.
Open MRS entries of patient and family members.
Referral of patient and family members to MO for investigation.
Monthly follow up with patients for medicine and check up.
Registration of diagnosed TB patients in TB-03 with complete protocol.
Preparation and submission of weekly progress reports
