• Collection, preparation, and review of patient’s files that are discharged from the hospital.
  • To get all verified diagnostic reports and log as per MR procedure.
  • To provide CMR (Computerized Medical Record) to all those who are authorized to have access for MR files.
  • To deal and update queries of customers/ patients/ Hospital staff regarding Medical Record files.
  • Involvement in audits and repairing all damaged/ torn files.
  • Supervises leave and attendance roaster of the staff.
  • Assists medical staff by answering inquiries and locating patient records; retrieves requested data.
  • Provides technical assistance to medical records staff on questions involving forms, procedures, and record retention.
  • Participates in the design of patient information systems involving, receiving, storing, processing, and communication of data.
  • Evaluates the indexes and other component parts of the filing systems.
  • Monitors chart entries to ensure accuracy and completeness.
  • Serves as department liaison for outside parties with inquiries regarding departmental policies and procedures.
  • Assists in the production of forms, records, and reports reflecting Medical Records statistics and patient data.
  • Ensures that medical dictation is coordinated with correct patient record.
  • Conducts chart analysis, makes corrections, and coordinates record investigations and reconciliations.
  • Ensures compilation and provision of all the necessary documents for retrieval of claims from the SSP and Corporate patients.
