InterOp Care is an organization focused on primary health care in North America. We are hiring physicians who would be responsible for medical guidance and directing the clinical program focused on longitudinal care of the patients remotely.
Responsibilities, but not limited to:
- Monitoring and coordinating treatment and care plans. Educating patients about their medical conditions.
- Connecting patients with healthcare providers and evaluate their progress.
- Updating patient-related health information, insurance, and financial information.
- Ensuring all the immediate patient's concerns and requests are being addressed.
- Assessing and developing health care interventions with the care team.
- Working in collaboration with other healthcare providers and care teams including behavioral health, disease care management, home health care, social work, and community-based organizations.
- Tracks applicable laws and ensure compliance.
- Training as required will be provided.
