Dispensing, dose adjustment and reconstitution of chemotherapy medication according to patient needs in oncology department.
- Clinical checking and validation of prescription.
- Dose calculations adjustments, dilutions and stabilities, drug interactions (drug-drug, drug-food) check.
- Provide comprehensive clinical pharmacy service, working closely with pharmacy oncology team.
- Patient counseling and education.
- Supervise the procurement, preparation and dispensing of chemotherapy.
- Training of nursing staff about preparation and handling of cytotoxic medicine.
- Maintaining patients e record and SPSS data.
- Recording for adverse drug reaction, infusion relates reaction or HSRs and providing possible ideas or solution or management plans to minimize reactions for patient care and compliance.
