Our client, Kindle Care, is seeking a Patient Care Coordinator based in Lahore. The ideal candidate will hold a BS degree in Psychology or English and possess strong writing skills. This role involves managing and maintaining all documentation related to the support and care of clients with mental health needs. The coordinator will prepare and regularly update care plans, risk assessments, activity and meal planners, support plans, and other essential documents that contribute to the clients’ overall health, wellbeing, and daily support. Effective coordination with other teams via email is also a key part of the role.

The successful candidate must demonstrate excellent written and verbal communication skills in English, have a keen eye for detail, and be capable of organizing their workload efficiently to meet deadlines. Confidence in handling detailed documentation and maintaining accurate, up-to-date records is essential. While previous experience in all aspects of the role is not mandatory, full training will be provided to ensure the candidate is well-prepared.

Key Responsibilities

- Coordinate comprehensive patient care plans by collaborating with healthcare providers, patients, and families to ensure seamless integration of services.
- Develop, implement, and monitor individualized care plans tailored to patient needs, preferences, and health goals.
- Facilitate communication and cooperation among interdisciplinary teams to promote smooth patient care delivery.
- Manage case documentation, ensuring accurate and timely records of all patient interactions and care activities.
- Provide health education and guidance to patients and their families to improve understanding and engagement in care processes.
- Assess patients’ health literacy levels and adapt education efforts to enhance health outcomes.
- Organize patient care workflows to optimize efficiency and minimize delays in service delivery.
- Identify barriers to care and collaborate proactively with healthcare teams to resolve issues promptly.
- Serve as a liaison between patients, families, and healthcare providers to support consistent and compassionate care.
- Monitor patient progress and adjust care plans in partnership with clinical staff to address evolving health needs.
- Ensure compliance with healthcare policies, standards, and ethical practices in all coordination activities.
- Support quality improvement initiatives aimed at enhancing patient safety and satisfaction.
- Maintain clear and thorough documentation and communication channels to keep all stakeholders informed about patient status and care plans.

Required Qualifications

- BS degree in Psychology or English.
