You will be responsible for overseeing drug inventory, monitoring medication orders, and participating in specialized rounds such as antibiotic stewardship. Your contributions will directly impact patient safety, medication efficacy, and the overall quality of healthcare delivery.

Key Responsibilities

- Participate in clinical rounds alongside consultants and coordinate with floor doctors and staff to ensure seamless pharmacy support.
- Supervise and assist in managing pharmacy inventory, collecting necessary data for interventions.
- Review, monitor, and control drug profiles and Computerized Physician’s Order Entry (CPOE) for all admitted patients, including those in ICU, CCU, and other critical care units.
- Communicate with physicians regarding any inaccuracies in medication orders, unavailability of drugs, or suitable alternatives.
- Engage in antibiotic stewardship rounds to enhance the pharmacist’s role in patient care.
- Monitor the dispensation of high-alert medications, providing essential precautions and administration information.
- Document all pharmacy interventions in the system and report relevant issues daily to the Manager of Pharmacy Services.
- Compile and prepare monthly presentations on effective pharmacy interventions.
- Assist the Manager of Pharmacy Services in developing and implementing the hospital formulary.
- Foster strong communication channels between pharmacy departments and floor staff.
- Identify and resolve issues related to the Hospital Management Information System (HMIS) in coordination with the Health Informatics team, providing summary reports to management.
- Review and monitor stock levels of crash trolley medicines, submitting monthly reports to the Pharmacy Services Manager.
- Provide accurate and current drug information to healthcare professionals, including updates on new medications and formulary agents suitable for specific therapies.
- Record drug information data and prepare graphical monthly reports for the pharmacy department.
- Offer technical and administrative support to pharmacists, pharmacy technicians, and interns, including training.
- Counsel doctors, staff, patients, and their families on the safe and effective use of medications.
- Organize Continuing Medical Education (CME) programs for pharmacists, interns, technicians, nurses, and paramedical staff focused on the rational use of drugs.
- Monitor and document all non-formulary drugs arranged for inpatients.
- Perform additional tasks as assigned by the Manager or Assistant Manager of Pharmacy Services.

Required Qualifications

- Proven experience in clinical pharmacy services within a hospital setting.
- Strong knowledge of drug inventory management and medication safety protocols.
- Familiarity with Computerized Physician’s Order Entry (CPOE) systems and Hospital Management Information Systems (HMIS).
- Excellent communication skills to liaise effectively with multidisciplinary healthcare teams.
- Ability to document and report pharmacy interventions accurately.
- Competence in organizing educational programs and training sessions.
- Commitment to patient safety and adherence to ethical standards in healthcare.
