I, Muhammad Yakoob is graduate and working as Supervisor DFCs (District Field Coordinators) at Provincial Level in Zindagi Mehfooz Electronic Immunization Registry Program (ZM-EIR) organized by Interactive Research and Development (IRD) and The Indus Health Network (IHN)-Global Health Directorate (GHD) in collaboration with Expanded Program on Immunization (EPI), Health Department Government of Sindh. I have worked as OTP Assistant in Nutrition program by NGO named Health, Education & Literacy Program (HELP) funded by UNICEF in Naushahro Feroze District. I had also worked with different organizations in health education and agriculture sector. Dynamic and motivated professional with a proven record of Routine Immunization, Nutrition & Agricultural Management, Skilled in building cross-functional teams, demonstrating exceptional communication skills, and making critical decisions during challenges. An adaptable and transformational leader with an ability to work independently, creating effective presentations, and developing opportunities that further establish organizational goals. I have also well experience in Computer Networking, Ms office & Typing certified, Android application, GIS & GSM Tracking System.

Provincial Supervisor
Assist training and monitoring teams on data collection.
Assist in arranging meetings and trainings.
Data analysis using SPSS and Stata.
Supervise, mobilize and manage field teams. Establish and maintain relationships with partners and stakeholders i.e. Government, community focal persons, political and social activists and different NGOs, etc.
Prepare presentation slides for the managers meeting.
To notify the manager, IT Team and take corrective actions whenever any irregularities are suspected or detected.
Optimize the practical use of data collection tools, both paper and App based.
District Field Coordinator (DFC)
To support, supervise and monitor field activities of the Field Workers/Supervisors and Vaccinators.
To coordinate, support, supervise and Monitor field workers/supervisors in solving field issues.
To assess the training needs of the vaccination staff (Vaccinators and LHWs) and Field Supervisors as well as help them in provision of training on vaccination mobile application and provide support/facilitation, as and when required.
To conduct community awareness sessions.
To validate data received from field workers/vaccinators.
To participate in referral chain process for Zindagi Mehfooz Immunization for increasing the number of clients to get vaccinated on time.
To actively participate in any district government meetings regarding the Immunization/Polio and also liaison with District Health Administration.
To prepare daily field reports and share with Manager for monitoring purposes.

OTP Assistant
• Ensure the proper screening, assessment and registration of children (6 to 23 months) as per UNICEF protocol.
• Assist the Nutrition Assistant in providing medical and nutritional treatment for malnourished children at OTP site.
• Provide follow ups of admitted SAM & MAM cases.
• Ensure Proper care, packing and distribution of nutrition commodities to the registered and deserving patients.
• Ensure the timely referral of severely acute malnourished children with complications to the stabilization center.
• Ensure timely submission of reports.
• To counsel all the discharged patients for further follow ups through Social Mobilizer(SM).
• Be flexible and adaptable with regards to the implementation of the daily work.
Social Mobilizer
To collect the information required during assessments and surveys;
