the medication I evaluate the efficacy and safety of each medication based on patientspecific factors , including goals of therapy , adverse effects, and sign and symptoms of an
allergic reaction . In IPD and OPD department I have been involved in various areas
ranging from medication reconciliation , antibiotics usage , adverse drug reactions , drug
drug interactions , presence in code blue (cardiac arrest) situations , venous
thromboembolic disease prophylaxis,assistance in compliance with guidelines , curtailing
cost, prevention of medication error , and resident education .
