·     Plan and organize OTP/CMAM activities at designated distribution sites in liaison with health facility Incharge. Work closely with DHO Health staffs to ensure all aspects of the program understood and appropriate referrals take place to and from the CMAM outreach and well integrated into the Primary HealthCare and CMAM approach.
·     Ensure correct clinical diagnosis at the time anthropometric assessment so that        the SAM patient receives appropriate treatment and enrollment based OTP             admission criteria.
·     Timely identification of complications,non-responder,and referrals to the NSC and strong follow up until discharge for re-admission in OTP.
·     Provide success stories/case studies in the intervention with coordination of      Health facility incharge,Nutrition Officer /Manager.
·     Complain with National CMAM Guideline OTP protocol strictly for SAM case  management.
Performance other duties requested by the immediate supervisor
   

.Plan and organise CMAM activities (treatment of 6-23 months childern) at designated distribution sites in liaison with health facility Incharge and District Nutrition Officer.
Work closely with ACF and DoH s to ensure all aspects of the programme are understood and agreed and appropriate referrals take place to and from the CMAM and the programme is well integrated into the health systems.
Supervise the activities of the community groups (CNVs, MTMSG, PLW/Caregiver groups and TBAs)
Work closely with the health facility staff and LHWs to ensure all children are screened correctly at health facility/CMAM center and within targeted area, also to identity CBA in the intervention areas.
Ensure all criteria are respected and understood by the team and community groups, including admission, discharge and transfer to and from the CMAM.
Ensure comprehensive health checks and treatment according to the protocols.
Enusre correct clinical diagnosis at the time of assessment so that the patient receives appropriate treament in the relavant program.
Timely identification of complications, non responder, and referrals to the SC and strong followup uptill discharge for re-admission in OTP.
Review the forms completed by health facility staff and community groups to ensure completion, accuracy and coherence.
Ensure effective IYCF services are provided with Coordination of Health Staff as per National protocol guided in Pakistan IYCF strategy .
Ensure adequate availability of all medical and food supplies and ensure stock is pre-positioned as planned by discussing with logistics.
Ensure the MM Supplementation to children and PLW.
Report on progress and any problems in a timely manner to OTP Taulka Supervisor and DM/ Project Officer.
Ensure cases not meeting admission criteria return home as early as possible and understand why their child is not admitted
Through liaison with the community groups, provide effective follow up of all cases: particularly tracing of children who are absent from the programme, sick children, long stay children, children not gaining weight, and children with social problems etc.
Provide key health and education messages to beneficiaries based the protocol and guidelines

Responsibility to ensure the effective implementation of CMAM community outreach activities in the field.
Develop micro plan of community outreach activities and submit on monthly basis.
Community sensitization on the program objectives and methods to stimulate & eventually promote the understanding and participation of the target population. Community sensitization starts at village- and Community-leader levels and eventually extends to the entire population.
Establishment of community outreach setup at each union council through selection of community volunteers and health workers.
Community mobilization and beneficiary screening, provision of MM Supplementation and follow up.
Establishment of strong referral mechanism between community to health facility OTP/CMAMI site and follow up.
Establishment of nutrition community support groups (MTMSG,FTFSG, CVG) at each union council in consultation with community.
Support to the Community Health Worker (CHW) and Volunteers during screening (active case finding) and linking up the MTMSG , FTFSGs with the LHW program
Provide technical support to CHW on mobilization, active case finding, defaulter tracing and home follow up visits through follow up trainings and on job coaching and supportive supervision
Ensure community social behavior change communication health and awareness session/campaign on regular basis through CHW/support groups.
Establish linked and meetings with the Community leaders and different group existing in the working 

. Ensure polio compagin conducted in accordance with regularatory requirements.
.Ensure that all candidate have an equal opportunity to demonstrate their abilities.
.Give all their to conducting the Polio compagin to attend UPEC meeting.
Team visit pre &Post compagin monitoring.

Report to polio immunization officeer & area Coordinator

Plan for the field data collection exercise
Oversee and supervise the data collection exercise
Assures Quality of the collected data for completeness and accuracy.
Generate values for rate administration through the computer aided valuation system.
Preparing activity progress reports
Provide feedback and take immediate notice of and resolve any problems arising in the field.
Plan and prepare requirements for the field exercise.
Sensitize stakeholders
Coordinate and supervise all field activities.
Review collected data attributes for completeness and accuracy.
Plan for field validation to ensure completeness of the data collection exercise
Ensure data collection is comprehensive and as per the SOPs of the Organization
Train field data enumerators in the area of data collection.
Compile and submit reports
Any other duty not listed but assigned by higher management

Assist senior staff in work planning and door to door surveys at all levels.
Create awareness of project aim and objectives in the communities
Motivate community people through orientation sessions and project briefs.
Responsible for reporting and maintaining the record updated on daily basis.
Good in communication and flexible, able to present organization on different platforms.
As team member, skills to motivate the community people and team members to develop project activities, planning and reporting.
Identification and involvement of the vulnerable groups and families in the project planning and implementation.
Facilitate the IEC material distribution among the communities
Any other activity assigned by senior
