Medical Coder

ClinTech Solutions Pvt Ltd is seeking a detail-oriented Medical Coder to join their healthcare team. The successful candidate will be responsible for accurately analyzing clinical documents, assigning appropriate medical codes, and maintaining comprehensive electronic health records. This role requires a strong understanding of medical terminology, healthcare compliance standards, and coding protocols such as SNOMED. The Medical Coder works collaboratively with clinicians and other healthcare professionals to ensure accuracy and completeness of patient documentation while adhering to organizational policies including confidentiality under GDPR and HIPAA. The Medical Coder supports quality assurance initiatives by participating in audits and producing regular coding reports. This position demands meticulous attention to detail, strong organizational skills, and the ability to handle sensitive health information responsibly. The role is critical in maintaining the integrity and efficiency of patient record management and supports overall healthcare service delivery without direct team management responsibilities.
Responsibilities

Analyze and process clinical documents to extract and record precise patient information.
Identify missing or incomplete data within patient records and work with clinical staff to resolve discrepancies.
Assign industry-standard codes such as SNOMED to clinical documents accurately according to coding protocols.
Collaborate effectively with clinicians and healthcare professionals to verify the accuracy and completeness of medical records.
Enter detailed correspondence and data into electronic health records (EHR) systems, ensuring compliance with QOF, PCN DES, and other key performance indicators.
Prepare routine and ad hoc coding progress reports and present findings to Directors and Team Leads.
Participate actively in audits, quality improvement initiatives, and Significant Event Analysis (SEA) reviews to enhance coding accuracy and workflow efficiency.
Maintain strict confidentiality and handle sensitive patient data according to GDPR and organizational guidelines.
Promote workplace health and safety by following established protocols and reporting hazards promptly.
