We are seeking a dedicated full-time Case Worker / Patient Support Coordinator to join our remote team based in Pakistan. This role involves providing essential support to U.S. patients by facilitating access to healthcare, social services, and community resources. The position requires effective communication and coordination with various stakeholders, including healthcare providers, insurers, government agencies, and community organizations. The role operates during night shifts aligned with New York business hours, offering a competitive monthly salary.
Key Responsibilities
- Conduct telephone communication with patients to assess needs and provide support.
- Navigate and connect patients to appropriate healthcare, social-service, and community resources.
- Coordinate with healthcare providers, insurance companies, government entities, and community organizations to ensure seamless patient support.
- Maintain accurate and timely documentation of patient interactions and case progress.
- Advocate for patients by addressing barriers to care and facilitating access to necessary services.
- Collaborate with internal teams to improve patient experience and resource utilization.
Required Qualifications
- Proven experience in case management, patient support, social work, or a related field.
- Strong communication skills, particularly over the phone, with the ability to engage and empathize with patients.
- Ability to navigate complex healthcare and social service systems effectively.
- Excellent organizational skills and attention to detail for managing multiple cases simultaneously.
- Comfortable working night shifts aligned with U.S. Eastern Time (New York business hours).
- Proficiency in English, both written and spoken, to communicate with U.S.-based patients and partners.
- Reliable internet connection and a suitable remote work environment.
