Patient Care Coordinator

Revex Square is seeking a dedicated and enthusiastic Patient Care Coordinator to join our expanding healthcare team in Islamabad. This full-time, onsite role is ideal for individuals with excellent communication skills and a patient-centered approach who are passionate about the US healthcare industry. The position involves working night shifts from 6:00 PM to 3:00 AM at our Ghauri Town, Fawara Chowk location. As a Patient Care Coordinator, you will play a vital role in supporting US-based patients by facilitating their healthcare journey and ensuring a smooth and compassionate experience.

Key Responsibilities
- Communicate effectively with US-based patients through inbound and outbound calls.
- Coordinate patient enrollment, follow-ups, and other care-related activities.
- Educate patients about healthcare programs such as Remote Patient Monitoring (RPM) and Chronic Care Management (CCM).
- Obtain and accurately document patient consent forms.
- Maintain and update patient records within the healthcare system.
- Collaborate closely with healthcare providers and internal teams to deliver seamless patient care.
- Ensure patient confidentiality and adhere strictly to HIPAA guidelines.

Required Qualifications
- Strong English communication and interpersonal skills are essential.
- Previous experience in US healthcare, patient coordination, or call center operations is preferred.
- Familiarity with RPM, CCM, or patient enrollment processes is a plus.
- Excellent organizational skills with keen attention to detail.
- Ability to work independently and efficiently in a fast-paced environment.
