Personic Health, a US-based in-home wound care center, is seeking a highly organized and tech-savvy Patient Care Coordinator to support its expanding operations. This role plays a vital part in ensuring patients progress smoothly through their care plans by coordinating efforts between internal clinical teams and external partners. The ideal candidate will manage patient scheduling, follow-ups, and communication to provide a seamless and efficient patient experience from the initial visit through ongoing care.
Key Responsibilities
- Serve as the central point of coordination for patient care, collaborating with clinicians, internal support teams, and external vendors to ensure continuity and quality of care.
- Manage patient scheduling and routing end-to-end, optimizing provider routes and visit timing to maximize efficiency.
- Handle real-time schedule adjustments and urgent updates throughout the day to maintain smooth operations.
- Conduct high-volume follow-ups to prevent gaps in care or communication, ensuring patients receive timely attention.
- Accurately document all patient interactions in real time across multiple systems, including Electronic Medical Records (EMR), Customer Relationship Management (CRM), and scheduling and mapping tools.
- Maintain data accuracy to support operational reporting and efficiency.
- Collaborate closely with internal teams such as nurse practitioners, wound nurses, wound technicians, data/intake teams, scribes, and support staff.
- Coordinate with external partners including home health agencies, durable medical equipment (DME) providers, mobile labs, and ancillary vendors.
- Proactively follow up on orders, referrals, and patient status updates to ensure smooth care transitions.
Required Qualifications
- Bachelor’s degree in healthcare, Health Information Management, or a related field.
- Proven experience in patient care coordination, scheduling, or healthcare operations.
- Strong communication and interpersonal skills to effectively liaise with diverse teams and external partners.
- Exceptional organizational skills with a keen attention to detail and a strong sense of accountability.
- Ability to thrive in a fast-paced, high-volume environment.
- Tech-savvy with demonstrated experience using multiple systems simultaneously.
Role Details
This is a full-time, onsite position requiring 40 hours per week. The shift is scheduled from 6:30 PM to 2:30 AM. The role is based in Karachi, Pakistan.
