This role involves overseeing and supporting pharmacy services related to inventory management and medication safety across primary care sites. The incumbent will be responsible for reviewing and monitoring patient drug profiles and computerized physician orders, ensuring accuracy and availability of medications. Collaboration with physicians and healthcare teams is essential to optimize patient care, particularly through participation in antibiotic stewardship rounds and management of high-alert medications. The position also includes documentation, reporting, and communication duties to maintain effective pharmacy operations and support clinical decision-making.
Key Responsibilities
- Supervise and assist in managing pharmacy inventory and collect necessary data for clinical interventions.
- Review, monitor, and control drug profiles and Computerized Physician Order Entry (CPOE) for all patients receiving medications at primary care locations.
- Communicate with physicians to resolve issues related to inaccurate orders, medication unavailability, or alternatives.
- Participate actively in antibiotic stewardship rounds to enhance the pharmacist’s role in patient care.
- Monitor dispensing of high-alert medications, ensuring all precautions and administration guidelines are followed.
- Document all interventions in the system and report relevant issues daily to the Pharmacy Services Manager.
- Compile and prepare monthly presentations on effective clinical interventions.
- Foster strong communication channels between pharmacies and physicians at primary care sites.
- Identify and resolve Health Management Information System (HMIS) issues in coordination with Health Informatics, providing summary reports to management.
- Review and monitor stock levels of crash trolley medicines and submit monthly reports to the Pharmacy Services Manager.
- Provide accurate and current drug information to healthcare professionals, including updates on new medications and formulary agents suitable for specific therapies.
- Record drug information data and prepare graphical monthly reports for the Pharmacy department.
- Offer technical and administrative support to pharmacists, pharmacy technicians, and provide training to interns.
- Counsel doctors, staff, patients, and families on the safe and rational use of medications.
- Organize Continuing Medical Education (CME) programs for pharmacists, interns, technicians, nurses, and paramedical staff focused on rational drug use.
- Monitor and document all non-formulary drugs arranged for inpatients.
Required Qualifications
- Relevant degree or diploma in Pharmacy or Pharmaceutical Sciences.
- Experience in clinical pharmacy services, including medication review and inventory management.
- Proficiency in using computerized physician order entry systems and health information management systems.
- Strong communication skills to liaise effectively with physicians, healthcare teams, and patients.
- Ability to document and report clinical interventions accurately.
- Knowledge of antibiotic stewardship principles and high-alert medication protocols.
