We are seeking a dedicated Pharm-D professional with 1-2 years of experience working on grant-funded projects in a similar capacity. The ideal candidate will play a vital role in ensuring the safe and effective management of pharmacy services, including medication dispensing, inventory control, and pharmacovigilance. This position requires close collaboration with clinical teams to promote rational drug use and provide accurate drug information, while maintaining compliance with established protocols and standards.

Key Responsibilities

- Ensure proper handover protocols are followed and submit daily reports to the Assistant Manager or Manager of Pharmacy Services.
- Cross-verify medicines issued and consumption data, correlating this information with clinical records.
- Develop and maintain an effective pharmacovigilance system to monitor drug safety.
- Accurately dispense medicines, review computerized physician order entry (CPOE) to prevent drug interactions, and counsel patients to promote rational drug use.
- Communicate with physicians regarding unavailable medicines and suggest appropriate alternatives.
- Ensure requisitions for medicines are issued accurately and timely.
- Monitor and manage inventory stock levels, preparing and forwarding demand requests to the Pharmacy Manager as needed.
- Receive incoming goods, conduct physical inspections, and store stock correctly, ensuring proper temperature conditions are maintained.
- Provide current and accurate drug information to healthcare professionals upon request, maintaining records in accordance with established protocols.
- Prepare monthly reports for the Assistant Manager/Manager Pharmacy detailing stock status, including fast-moving, slow-moving, short expiry, expired, broken, and damaged items.
- Label high-alert medications clearly, providing all necessary precautions and administration information.
- Document all interventions, drug information, and related activities, and report these details to the Assistant Manager/Manager.
- Monitor storage temperatures regularly and maintain accurate documentation of these checks.
- Provide management with regular updates and monthly Key Performance Indicator (KPI) reports.
- Perform additional tasks as assigned by the Manager or Assistant Manager of Pharmacy Services.

Required Qualifications

- Doctor of Pharmacy (Pharm-D) degree.
- Minimum of 1-2 years of relevant experience working on grant-funded projects in a similar role.
