Care Planning Pharmacist

We are looking for a dedicated and detail-oriented Care Planning Pharmacist to join our team at our main location on Shahra-e-Faisal, Karachi. This full-time, onsite role operates from Monday to Friday, with shift timings from 6:00 PM to 3:00 AM PKT. The successful candidate will play a vital role in conducting patient assessments, managing medications, and supporting individuals with chronic and complex health conditions through comprehensive care planning and effective communication.

Key Responsibilities:
- Conduct assessments related to Depression, Cognitive Impairment, and Medication Management.
- Review patient profiles thoroughly and document findings according to clinical criteria.
- Communicate with patients through outbound and inbound calls to explain care plans and address any questions.
- Develop and maintain Chronic Care Management (CCM) plans, including tracking medications, setting goals, monitoring vitals and lab results, and ensuring accurate documentation.
- Coordinate appointment scheduling and billing processes in collaboration with healthcare providers.

Required Qualifications:
- Bachelor’s degree or Doctor of Pharmacy (Pharm-D) degree.
- Strong understanding of chronic disease management and patient counseling techniques.
- Excellent communication skills, particularly in English, along with strong analytical abilities.
- Prior experience in clinical or care planning roles is preferred but not mandatory.
