Qualification: BS Psychology, BS English 

The candidate must have strong writing skills  

The Care Coordinator will be responsible for completing and maintaining all documentation relating to the support and care of clients with mental health needs.

The role will include preparing and regularly reviewing care plans, risk assessments, activity planners, meal planners, support plans, and other relevant documentation relating to clients’ health, wellbeing, and daily support. It also includes coordination with other teams via email. 

The successful candidate will have excellent written and verbal English communication skills, strong attention to detail, and the ability to organize their workload and meet deadlines. They should be confident working with detailed documentation and maintaining accurate and up-to-date records.

Full training will be provided before the individual takes on the role, so previous experience in all aspects of the position is not essential. 


Responsibilities
  • Coordinate comprehensive patient care plans by collaborating with healthcare providers, patients, and families to ensure integration of services.
  • Develop, implement, and monitor individualized care plans that address patient needs, preferences, and health goals.
  • Facilitate communication and cooperation among interdisciplinary teams to promote seamless patient care.
  • Manage case documentation and ensure accurate and timely records for all patient interactions and care activities.
  • Provide health education and guidance to patients and their families to enhance understanding and engagement in care processes.
  • Assess patients’ health literacy levels and tailor education efforts accordingly to improve health outcomes.
  • Organize patient care workflows to optimize efficiency and reduce delays in service delivery.
  • Identify barriers to care and work proactively with healthcare teams to resolve issues promptly.
  • Serve as a liaison between patients, families, and healthcare providers to support consistent and compassionate care.
  • Monitor patient progress and modify care plans in collaboration with clinical staff to address changing health needs.
  • Ensure compliance with healthcare policies, standards, and ethical practices in all coordination activities.
  • Support quality improvement initiatives aimed at enhancing patient safety and satisfaction.
  • Maintain strong, clear documentation and communication channels to ensure all stakeholders are informed about patient status and care plans.
