Physician Assistant

Responsibilities

Assist supervising physicians by coordinating patient scheduling and managing appointment logistics.
Review and analyze patient medical records and clinical documentation for accurate diagnosis and treatment planning.
Support the development and execution of individualized patient care plans under physician supervision.
Continuously monitor remotely transmitted patient health data, identifying and reporting significant clinical findings promptly.
Conduct follow-up communications with patients to ensure effective care coordination and adherence to remote patient monitoring and chronic care management services.
Document all patient interactions, clinical assessments, and coordination activities accurately within the electronic medical record system.
Educate patients regarding chronic disease management, medication compliance, and the correct use of remote monitoring devices.
Collaborate with physicians and multidisciplinary healthcare professionals to maintain continuity and high quality of patient care.
Ensure compliance with HIPAA regulations, healthcare policies, and all applicable organizational and legal standards.
Manage EHR-based care plan monitoring, prescription order entry, and referral management to support timely and effective patient care delivery.
